STEP 2
LEADER FORM: Youth Pastors and Adult Leaders Form

NOTE: Each group must have at least 2, non related, adult leaders.

Every Youth Pastor and Adult Leader participating in the Youth Mission Trip to New York City at NYSUM MUST complete this form. 

The information you provide will be used by Elim Fellowship to run the required background check for all adults attending the trip. Please fill out each section carefully to ensure we have accurate and complete information.

This costs Elim Fellowship $25 per person, but we have not included that in the price of the trip, as we consider this step essential for the safety of all our children.

REFERENCE FORM: Pastoral Reference Form

Your pastor will be invited to complete this form for each of the leaders participating in this trip. We consider this step essential for the safety of all our children.

Tuesday, March 31 - Morning & Afternoon Ministry, Evening Worship Service and Debrief

Group Requirements & Rooming Details

Groups must have 2 adult leaders who are not related and not from the same household in order to participate. NYSUM houses teams using gender-separated floors, and each group’s students will be placed accordingly.

Students do not need to have an adult staying in the room with them; however, if your group chooses to place adults in student rooms, two adult leaders are required in that room for accountability and safety.
(Please plan your team structure with this in mind to ensure proper supervision and compliance with our safeguarding guidelines.)
